Agreement and Release of Liability Form

 

In consideration for being allowed to participate in the activities and programs of Better Human Pilates LLC © I do hereby waive all responsibilities or liability for any (i) errors, mistakes, or inaccuracies of content; (ii) personal injury or property damage, of any nature whatsoever, resulting from the access to or use of the services provided at Better Human Pilates LLC; and (iii) any unauthorized access to or use of these secure servers and/or any and all personal information stored on the website.
I also hereby release Better Human Pilates LLC from any responsibility or liability for any injury or damage to myself, including those caused by the negligent act or omission of any kind of services arising out of or connected with my participation in any activities of Better Human Pilates LLC.

I understand and am aware that strength, flexibility and aerobic exercise, including the use of equipment, are potentially hazardous activities. I also understand that fitness activities involve the risk of injury and even death, and that I am voluntarily participating in these activities and using equipment and machinery with knowledge of the dangers involved. I understand this program does not provide any form of medical treatment, nor are its fitness professionals licensed medical practitioners. I hereby agree to expressly assume and accept any and all risks of injury or death.
